Please do not send medical history, health records, X-rays, insurance numbers, Social Security numbers, driver's license numbers, payment card details, or bank information through this site or its forms. We do not ask for that information in our standard matching form.

You are responsible for making sure the information you submit is accurate and belongs to you, or that you are authorized to submit it for another person. If your contact information changes, you should let us know so we can update our records where appropriate.

4. Matching Service and No Guarantee

If you request a match, we may share the contact and interest details you provide with participating oral and maxillofacial surgeons or their offices so they can contact you about a consultation. The matching service is free to the patient.

We do not guarantee that you will be matched with a surgeon, that any surgeon will contact you, or that any surgeon will accept your case, insurance, schedule, or location. We also do not guarantee availability, wait times, language services, pricing, financing, or treatment outcomes.

Any surgeon you choose is independent from Oral Surgery Path. You are responsible for evaluating the surgeon, confirming that the surgeon is board-certified, reviewing the treatment plan, understanding the risks and alternatives, and getting the full cost in writing before scheduling. A second opinion is always reasonable.

Educational content on this site may describe common procedures, recovery patterns, or cost ranges in general terms. Those ranges are not quotes, and typical timelines are not promises. Everyone heals differently, and the real price and timeline depend on the case, the surgeon, the facility, the sedation used, your area, and other factors discussed during a consultation.

When you submit a form or otherwise ask to be matched, you agree that Oral Surgery Path and participating surgeon offices may contact you using the contact information you provided. Contact may be by phone call, mobile call, voicemail, email, or text message, including through automated technology, to respond to your request and help connect you with a consultation.

You understand that your consent to receive these communications is not a condition of buying any goods or services. Oral Surgery Path does not charge patients for matching.

You can opt out at any time. For text messages, you can reply STOP if that option is offered. You may also use the unsubscribe method in an email, ask the calling office not to contact you again, or reach out through our Contact page.

Message and data rates may apply from your phone carrier. We are not responsible for delays or failures in calls, texts, or emails caused by carriers, spam filters, technical issues, or incorrect contact information you submitted.
